If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 8 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Travel Health Questionnaire
Please complete the questionnaire above and return to the surgery at least 8 weeks before you travel. If your travel is within 8 weeks please attend the travel clinic at Boots or alternative providers.
You can email the completed questionnaire to email@example.com or you can drop it in at reception. The nurses will review your form and ask reception to contact you and book you in for an appointment. If possible some vaccinations may be given at this initial appointment, but some vaccines may need to be ordered in so you may need a subsequent appointment. You may incur a charge for the vaccine - some are available on the NHS but some are private and you will need to pay for them. Payment can be arranged at reception.

After consultation and diagnosis with your G.P. you will be given a leaflet explaining this service and instructions on what you need to do. You will then be able to ring the Triage Service and have details taken and then be called back by a Physiotherapist at a time convenient to yourself. At the end of their telephone assessment (on average lasting 15 minutes) the Physiotherapist will either:-

The fees charged are based on the British Medical Association (BMA) suggested scales and our reception staff will be happy to advise you about them along with appointment availability.
