PRESCRIPTIONS
If it is a new medication you will usually need to see the GP. New patients are advised to make an appointment to meet one of the doctors and set up the repeat prescriptions system for their long term medication.
REPEAT PRESCRIPTIONS
If you are needing medication frequently or regularly this system enables you to obtain supplies with out having to see the GP every time. However it is our duty to ensure regular reviews are carried out and that the appropriate amounts are being given. You will be issued with a list of medications on repeat and a review date will be set (see prescription example)

Repeat prescriptions can be requested in many ways but NOT over the
telephone.
You can
- Hand them in at the surgery in the post box provided in the reception desk, post them in the main surgery letterbox or we also have a prescriptions box at the main gates to enable you to do this when we are closed.
- Use your Chemist - Almost all chemists now take requests over the telephone and organise your repeat prescription for you. Most chemists offer a collection service of your prescriptions. Please ask your chemist about this.
- Post your repeat slip with the items ticked that you require to the surgery, with a S.A.E if you wish them to be returned by post.

Go online and order
To use the Internet facility you need to request an access number and code from reception, this needs to be done in person at reception.
There are two online user screens. The information you enter on the first screen enables the system to check your identity when you log on. On the second screen you enter a password and security details that no-one else will know.
The information you need to complete the registration screens is shown below. Look after this form carefully until you register, and consider destroying it afterwards. If you lose the form or if it is stolen before you have entered your password and security details, contact the practice straight away, by telephone or in person.
Remember to enter the information exactly as it appears on this form, or your registration will not be accepted. When you log on after registering you must also enter all details in exactly the same format.

Enter the other information yourself. Remember your password and security information, but do not write them down in identifiable form.
- E-mail your request to mp.marysville@nhs.net (please include your name, DOB and your address).
- Fax: please tick all items that you require and any other instructions
Please remember to allow 48hours i.e. 2 working days, before coming to collect your prescriptions from the desk.
Please take note of any messages on the right side of the prescription as this is how we advise most patients that they are overdue for review of their medication.
Please speak to your chemist about the services they offer.
PLEASE NOTE it is the patients responsibility to inform the chemist
of their choice that there will be a prescription ready to collect.
REVIEW DATES
(at bottom of right hand page) Please make an appointment to see your GP, or practice nurse for blood pressure/asthma reviews, if your review date is overdue. You may not be issued with your prescription if you do not attend a review.OTHER PRESCRIPTION REQUESTS
We will accept requests for other medications particularly if you have been prescribed them in the past by one of the doctors at Marysville. This needs to be requested in writing or via fax or e-mail and NOT OVER THE PHONE. However in some situations the doctor may need more information and will not be able to issue the medication until you are seen or contacted.
