COVID-19 – Vaccination Status/Proof
The quickest and most effective way of obtaining your vaccination details is to register for an account on https://www.patientaccess.com – from this site you can “Share” or “Export” your vaccination status information. For example you can “Export” to a PDF, which will show your name, DOB, address and list your vaccinations. Please see this redacted copy example. This is a free and fully protected website, affiliated with the NHS.
Please note that you should allow the system at least one week from your vaccination date(s) to be updated with your vaccination information.
If you still require or would prefer a GP letter of proof of vaccine, please be advised that this is chargeable, with payment in advance. We accept cash, cheque, BACS or PayPal payments.
The letter can either be emailed or posted to you. As above you would need to allow at least one week from the date of the vaccine for the information to come through to the practice.
