New Patient Registration

To register with the practice patients are requested to complete a registration form, which is available from reception or can be downloaded here:

When you bring it in to reception please bring some ID with you and we can get you set up to access services online as well.

You will also need to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. Please click the appropraite links below, print, fill out and bring with you to the practice:

If you do not have a printer, both of these forms are available at reception.

The receptionist will make you an appointment with the practice nurse, who will then complete the registration process. We have disabled access to all the rooms in the surgery.

You have the right to request to be seen by a particular clinician and such requests will be recorded in your notes. This choice can be changed but we recommend that you stay with the same clinician once a course of treatment has started.

If you are not fluent in English please inform the receptionist when registering and please ensure that you bring an interpreter to all consultations.

Changes of Personal Details

Please inform reception if you change your name, status, address or telephone number. You will be asked to complete a form to ensure that your records contain up to date information.
